ADMISSION REQUIREMENTS
For incoming NEW and TRANSFEREE College and Senior High School enrollees, kindly prepare the listed requirements in both hard and soft formats for submission to our online enrollment portal and the Registrar's Office.
Basic Requirements:
- SF-9 (Form 138 or Report Card)
- SF-10 (Form 137 or Permanent Record)
- Good Moral Certificate
- PSA Birth Certificate (Photocopy)
- Two pieces 1x1 Colored ID Pictures (Plain White Background)
Additional Requirements:
- For Alternative Learning System (ALS) Passers: AF5 or Learners Permanent Record
- For Transferees: Transcript of Records and Honorable Dismissal
